2009–10 Mills College Financial Aid Form

     To be filed in addition to the FAFSA.  If you are   
     unable to view this form, please call 510.430.2000 or
     email finaid@mills.edu to arrange to have it mailed.

2009–10 Cal Grant GPA Verification Form

     For Undergraduate California Residents Only

2009–10 Noncustodial Parent's Statement

     For New Undergraduates Only

     To be filed by the noncustodial parent in the case of 
     a dependent student whose parents are divorced, 
     separated, or were never married. (The custodial  
     parent should complete the FAFSA and Mills College
     Financial Aid Form.) The Noncustodial Parent's
     Statement should be submitted to the M Center
     at Mills.

2009-10 Certification of Non-filing form

     For Continuing Undergraduates Only

     To be filed by independent undergraduate students
     who are not required to file a 2008 federal tax 
     return and by the parent of a dependent 
     undergraduate student if the parent is not required
     to file a 2008 federal tax return.  This form should be
     submitted to the M Center at Mills.

2009-10 Dependent Verification Worksheet

     If you are a dependent student, and your federal 
     Student Aid Report (SAR) indicates that you have
     been selected for federal verification, please
     complete this form and submit it to the M Center at
     Mills. 

2009-10 Independent Verification Worksheet

     If you are an independent student, and your federal  
     Student Aid Report (SAR) indicates that you have 
     been selected for federal verification, please complete
     this form and submit it to the M Center at Mills.

2009-10 Federal Stafford Loan Request Statement

     To be filed by students who have been awarded a 
     Federal Stafford Loan as part of their financial aid
     award.

2009-10 Federal PLUS Loan Request Statement

     To be filed by parents applying for a Federal PLUS
     Loan.  This form should be submitted to the M Center.

2009-10 Fed. Graduate PLUS Loan Request Statement

     To be filed by graduate students applying for a  
     Federal Graduate PLUS Loan.

2009-10 Federal Perkins Loan Request Statement

     To be filed by students who have been awarded a 
     Federal Perkins Loan as part of their financial aid
     award.

2009-10 Mills College Loan Documentation

     To be filed by students who have been awarded a
     Mills College Loan as part of their financial aid award.
